Wound Care

Diabetics must be wary of all wounds, regardless of depth or size. Diabetes, a chronic disease in which the body cannot properly use glucose the way it normally would, causes various complications that make wounds difficult to heal. Nerve damage or neuropathy will cause diabetics to have trouble feeling the pain of a blister or cut until the condition has significantly worsened or become infected. A diabetic’s weakened immune system can make even the most minor of wounds easily susceptible to infection. Diabetics are also more prone to developing narrow, clogged arteries, and are therefore more likely to develop wounds.

Wounds should be taken care of immediately after discovery, as even the smallest of wounds can become infected if enough bacteria build up within the wound.  To remove dirt, wounds should be first rinsed under running water only. Soap, hydrogen peroxide, or iodine can irritate the injury and should be avoided. To prevent infection, apply antibiotic ointment to the wound and cover it with a bandage. The bandage should be changed daily. The skin around the wound may be cleaned with soap.

To prevent further exacerbation, see a doctor—especially if you have diabetes. Minor skin conditions can become larger problems if not properly inspected. As the wound heals, make sure to avoid applying pressure to the affected area.

Limb Salvage

Limb salvage is a procedure that involves saving a lower extremity from amputation. In podiatry, limb amputation often occurs as a result of diabetes, peripheral vascular disease, neuropathy, cancer, and severe crush injury. The fundamental goal of limb salvage is to restore and maintain stability and movement of the affected lower extremity.

The procedure typically involves removing the diseased tissue and a small portion of the surrounding healthy tissue, as well as the removal of any affected bone if necessary. If the bone is removed, it is then replaced with prostheses, synthetic metal rods or plates, or grafts from either the patient’s body or a donor. Limb salvage is typically the preferred choice of procedure over amputation, as the procedure preserves both the patient’s appearance and allows for the greatest possible degree of function in the affected limb.

Types of Foot Deformities

Pronated feet, marked by an inward-leaning heel, may arise in childhood, potentially causing issues later in life, particularly in overweight or knock-kneed individuals. People who have high-arched feet, may shift their weight to the ball of the foot, leading to pain, calluses, and an increased risk of ankle injuries. Equinus foot, where the foot points downward and the heel cannot touch the floor due to shortened calf muscles, presents challenges in walking and standing. Clubfoot, a severe congenital deformity involves the foot pointing downward and inward, restricting normal standing and walking. Causes of foot deformities also include abnormal strain, tight footwear, injuries, inflammations, being overweight, and underlying diseases such as osteoarthritis or rheumatoid arthritis. Biomechanics in Podiatry

Podiatry is a branch of medicine that deals with the study, diagnosis, and treatment of foot and ankle conditions. There are various subdivisions in podiatry; biomechanics is one of them. Biomechanics is the way in which the bones, muscles, and joints of the feet and lower limb interact with each other.

Our feet play crucial roles in the way we move, and it is rare to have feet that are completely symmetrical. Common biomechanical issues include high or low arches or uneven leg heights. Excessive pronation often leads to fallen arches, or flat feet, and is a common cause of running injuries. People whose feet are over-pronated tend to have flexible and unstable feet. At times, people may be able to adapt to these abnormalities without any difficulties, but in other cases, these issues can cause a great deal of pain. Children’s Foot Blisters

Blisters, characterized by fluid-filled bubbles on the skin, often arise from friction or injuries like burns, spider bites, or pinching, and are occasionally due to infections. In most instances, managing blisters at home is feasible. Small, intact blisters, even blood blisters, generally heal naturally. Foot blisters in children should not be disturbed if unbroken. The exception is covering it with a bandage or moleskin pad and having the child avoid activities that provoke the blister formation. If your child has a blister that ruptures, persists, causes pain, or appears infected, it is suggested that you schedule an appointment with a podiatrist for proper care.

Prevention & Treatment

It is important to properly care for the affected area in order to prevent infection and ease the pain. Do not lance the blister and use a Band-Aid to provide pain relief. Blisters

Blisters are small pockets of fluid that occur on the top layers of the skin for several reasons. Friction, burns, and diseases are all known causes of blisters.  Smaller blisters are known as vesicles, while larger blisters are referred to as bulla. The fluid inside the bubble can be blood, pus, or serum; which is a clear liquid that protects the skin. In most cases, blisters are not a major health issue, but they can be an indicator of a more serious condition.  

Causes of blisters vary. Blisters are commonly caused by wearing poorly fitted shoes that rub against the foot. However, there are many other causes besides from friction; including burns, sunburn, insect bites, frostbite, poison ivy/oak, chemical exposure, impetigo, eczema, viral infections, and more.

Most blisters heal by themselves and do not require immediate medical care. If you have a blister, do not pop it since this may cause infection; it is advised to put a bandage over the blister to protect it. If the blister is large, causes pain, or if you have a fever, it is recommended that you see a doctor who can provide proper care. Blisters are easy to diagnose, and if considered prudent by the doctor, can easily be drained of fluid with a sterile needle as well.

To prevent blisters on the feet, wear shoes that fit properly and don’t cause rubbing. Socks can help prevent friction and it is recommended that you wear them if you are wearing shoes. Hand blisters can be avoided by wearing gloves during activities that cause friction against the hand. If you have a blister that pops, do not remove the dead skin, wash the area, apply antibiotic ointment, and cover with a bandage. It is okay in most cases to not seek immediate medical care for a blister if it was just caused by friction. However, if the blister causes pain or does not go away, it is suggested that you see a doctor for a diagnosis.

Causes and Treatment of Black Toenail

A toenail turning black could be more than just a cosmetic concern. Discoloration, particularly a black toenail, is often a sign of an underlying issue that may require attention. One of the primary reasons for a black toenail is a bruise beneath the nail caused by trauma or pressure, also called a subungual hematoma. This common occurrence can result from stubbing your toe or dropping a heavy object on it. It also is common among runners as a result of the repetitive impact of the toenail against the front of the shoe. Shoes that are too tight or poorly fitting may create friction, causing damage to the nail and leading to subungual hematomas. Further, fungal nail infections, particularly in individuals with compromised immune systems, diabetes, or vascular disease, can cause toenail discoloration. These infections thrive in moist environments, making sweaty feet more susceptible. Ingrown toenails, a painful condition where the nail grows into the surrounding skin, can also cause discoloration. This may lead to bacterial infections, contributing to the darkening of the nail. In rare cases, a black toenail may be a sign of subungual melanoma, a type of skin cancer. Although uncommon, it is essential to seek medical attention if you notice persistent discoloration that is not linked to trauma or fungal infections. Certain medications can lead to toenail discoloration, including cancer drugs, antibiotics, retinoids, and drugs used to treat autoimmune conditions. If you have noticed your toenail changing in color, it is suggested that you make an appointment with a podiatrist who can determine the cause, and offer treatment options.
